We currently have a vacancy for a qualified social worker to work within an Older Persons Swift Team. You must have post qualified experience, preferably within a hospital setting dealing with
safe and timely discharge. A good working knowledge of recent legislation's is needed along
with Fair Access to Care, Delayed Discharges Act, SAP and FACE. Please apply ASAP, only if you have a CRB within 12 months and your GSCC registration.
