Undertake the duties of a duty officer or its equivalent.
Apply Eligibility Procedure.
Signpost people to other services where appropriate.
Receive and respond to complaints within the written Complaints Procedure.
Undertake assessment of need – limited, comprehensive, supplementary, carers’, financial.
Identify risk and take appropriate action.
Register people under the CSDP Act.
Consult and negotiate with service users, carers and colleagues.
Develop and write care plans by defining objectives, identifying, prioritising and costing options.
Implement approved care plans by arranging services to meet identified needs.
Identify and record resource shortfalls.
Review and monitor care plans.
Act as an advocate where necessary and appropriate.
Provide support to carers.
Evaluate and develop own work.
Contribute to the effective working of a team and to professional development of others by demonstration, sharing knowledge and offering advice and support.
Contribute to the development of services.
Contribute to team and locality plans.
Network with other agencies.
Maintain records in line with procedures.
Respond to civil emergencies.
Using specific social work skills:-
Take responsibility for more complex work eg:-
Arrange Direct Payments.
Operate the Vulnerable Adult at Risk Joint Policy and carry out initial assessments following up more complex cases.
Participate in PACE interviews having received the necessary training.
Undertake the role of Approved Social Worker having received the necessary training.
Promote the development of services
Represent the Department’s and the Corporate view at meetings with other agencies.
