We are looking for a competent/able practitioner preferably with knowledge/experience of transition planning between childrens' service and adults.
You will need to be willing to learn quickly but will receive plenty of support from colleagues across services.
You will be required to assume immediate caseload. Transition is a significant priority and developmental area across services in Oldham. Very exciting time.
