Urgently required for up to 2 Months in the first instance. Two qualified social workers - must be GSCC registered and CRB cleared.
Duties will include Completing reviews of Continuing Care placements as part of the MDT and contributing to the completion of the Decision Support Tool in order to agree funding streams
As this is a Borough wide service, use of own transport would be an advantage and it is important that candidates are familiar with the use of CareFirst.
Training in their role in respect of the Continuing Care review will be arranged on taking up the post.
